If you are a current or aspiring full time student with a parent that has been seriously injured or killed on the job, we are here to help! Click here to review our scholarship guidelines to determine if you qualify. Please completed and submit the form and a a member of our Scholarship Committee will be in contact with you.
You will need to complete the entire application prior to submitting. We recommend you review/print this page and become familiar with all of the required information. Once you have gathered all of the information, then complete the application and submit using the button at the bottom.
It is important that all of the necessary documents are included with your scholarship application. In addition to completing the application below, please email the following documents to: Execdir@azkidschance.org.
High school transcript showing grades & attendance or if attending college, previous semester’s transcript. Minimum required GPA is 2.5.
Letters of recommendation; not provided by relatives.
Information on injured parent - description of accident, current rehabilitation reports, medical reports and ICA letter or death certificate.
Verification of school attendance
Copy of F.A.S.F.A. application/report
